What Is LTFC?
Long-Term Foster Care (LTFC) provides a stable, nurturing environment for children who are unable to reunite with their families in the near future. These children are placed in supportive family-based settings or group living arrangements where they receive personalized care, education, and emotional support.
Our LTFC program ensures a child-centered, trauma-informed approach, creating a safe and structured space where children can thrive while preparing for their next steps in life.
Who LTFC Serves
Our program is designed for children who:
Require a long-term family-based living arrangement to support their growth and development.
Are between the ages of 0 and 17.5 years at the time of placement.
May have special needs, be part of sibling groups, or be pregnant/parenting teens.
Benefit from culturally responsive care, including language and community support.
Key Features of LTFC
Our LTFC program offers a range of services designed to ensure the well-being and successful transition of UC into family and community-based settings.
Each child is assigned a dedicated case manager who oversees their care, legal status, and reunification efforts. Case managers work closely with attorneys, social workers, and foster families to support each child’s journey.
Children are enrolled in community-based schools to ensure academic continuity. Supplemental education, tutoring, and language support services are provided as needed.
Licensed clinicians provide individual and group counseling, trauma-informed care, and crisis intervention to help UC cope with past experiences and adjust to their new environment.
Children are placed in nurturing, family-based environments or supportive group settings that align with their cultural, linguistic, and emotional needs, ensuring they feel secure and valued
Each child receives guidance on their legal rights and options through experienced legal service providers. Our program ensures they have access to representation and support throughout any necessary legal processes.
UC receive medical, dental, and mental health care, including vaccinations, preventive care, and emergency services, ensuring their overall well-being.
How LTFC Works
The LTFC program at VisionQuest focuses on holistic care and individualized support. Key components include:
Children are carefully evaluated to determine their specific needs, strengths, and the best possible placement to support their growth.
Each child is placed in a family-like environment with dedicated staff supervision, ensuring their needs—such as age, background, language, and medical or emotional support—are met in a stable and supportive setting.
Caregivers provide continuous supervision and support, while children receive case management, counseling, education, and access to community resources to ensure a positive and nurturing environment.
Children attend public school while also receiving life skills training and personal development support, helping them build a strong foundation for the future.
As children grow, we provide guidance to help them transition into adulthood, higher education, employment, or other long-term living arrangements that support their success.
Evidence-based programs have been shown to successfully treat delinquent youth in the community and decrease out of home placement. Services cost between $1300 and $5,000 per family per year, while incarcerating just one youth will cost over $50,000 per year with the likelihood of poorer outcomes for both the youth & their family.

We provide 24/7 emergency support, ensuring that children and caregivers always have access to help when needed.
